New Patient Registration
If you live in our practice area and would like to register with us, please complete and sign a GPR REGISTRATION FORM to request registration at the practice. We routinely ask patients for some form of identification, for example photo identity such as your passport or driving licence and/or proof of address on a recent utilities or council tax bill.
If you have a Medical Card (Form GP4) this would also be helpful. Please complete all mandatory sections of the GPR form as this will help us to quickly obtain your medical records from your previous practice.
You will also be asked to complete a Patient Health Check Questionnaire to give us some key information about your past medical history. If for any reason your application is declined (this is very unlikely), we will write to you to explain the reason.
You have the right to express a preference to receive services from a particular clinician or group of clinicians within the Team, generally or in relation to a particular condition. If you express a preference, then we will record that preference in your medical records.
Your registration will be with the practice as a whole rather than with an individually named GP. However, you may request to see any doctor, and indeed it can be better if you can build a relationship with a particular doctor. You will understand that all the doctors are busy and that your preferred doctor may be fully booked on the day you wish to see him/her, but we will do our best to find an appointment to suit your needs.
Change of Address
Should you or any member of your family change your name, address, marital status or telephone number PLEASE REMEMBER TO NOTIFY US as soon as possible.
